Pager Transfer Information

If you are a direct customer of Spok (formerly known as USA Mobility) or your paging service is through an authorized reseller of Spok and you would like to transfer your paging service to DirectPage...

This form is required by Spok before a pager can be transferred.

If you are a direct customer of Spok, fill out the top portion of the form and fax back to DirectPage at 1-888-691-6491

If you are a customer of a Spok Reseller, you must first contact your reseller and they must agree to fill out this form for the transfer of your pager and pager phone number.

They are not required by law to transfer your number, however, if they agree to transfer your number, they must fill out the top portion of the transfer of ownership form, then fax back to DirectPage at 1-888-691-6491

Due to security reasons...When paying by Credit/Debit Card or Paypal...Both the Billing and Shipping addresses must be EXACTLY THE SAME.

If you have a different shipping address than where you receive your card statement, you may contact your card issuer and have them add the shipping address to your card account before placing your order. You can then enter your shipping address in both the billing and shipping address sections of your DirectPage account and the card should get approved.

We can ship to po boxes, so if you receive your card statement at a po box, simply select a US mail delivery option and list your po box as the shipping address.

If for any reason you enter two different addresses, your order will be canceled and the authorization hold may take up to 7 business days to fall off your payment account.
